At Hoffmann Foundation for Autism, we take meeting the needs of adults with Autism seriously. To ensure that we are able to fulfil this requirement we ask that a new referral form is completed to help us understand the needs of the person being referred so that we can identify how best we can help you

You can obtain a referral form in a number of ways which include the link on this website, via email or by contacting our Head office on 02073306110, so that we can send you a referral form in the post. You could complete the referral form and send it via email to referrals@hoffmannfoundation.org.uk or alternatively, send it via post to:

Once we have received your referral we will review the information provided and contact you to arrange a time when it is convenient for us to meet you so that we can undertake an assessment of your needs and to discuss how our services could benefit you and ultimately support you.

We aim to provide a prompt response to how we can support which we usually complete within a 28 day period.
